Request your personal information
To request your personal information you need to:
- Complete the Personal Information Request form.
- Provide 2 different types of identification. We cannot process your request until we've received the right ID:
- Proof of your name
- Proof of your address
- If you're making a request on behalf of someone else then you need to provide 2 additional types of ID as proof of their name and current address.
- Email your completed form and accompanying ID to us using the contact details on this page. Please do not send us your original documents. We can accept photos or scanned copies of your documents by email, as long as they're clear.
- If you're unable to email your documents, you can send us a copy by post which we'll securely destroy once we've verified your identity.
- You'll receive an acknowledgement as soon as we receive your completed form and ID.
- Once we've received all of your documents, we have a legal obligation to respond within 1 month. However, if your request is particularly complex then we can extend our response time by up to 2 months. If this is the case, we'll let you know as soon as possible.

Requesting information for someone over 11 years old
If the person you're requesting information for is over 11 years old, you must have their consent to make a Personal Information Request.
If the person is over 11 years old but doesn't have the mental capacity or understanding to provide consent, then an individual who holds parental responsibility or lasting power of attorney may act on their behalf if it's clear that the request is in the person's best interest.
Charges for making a request
It's free to make a Personal Information Request.
However, we may charge you a small fee if we feel the request is repetitive or excessive.
- A4 black and white: 5p each printed side
- A4 colour: 25p each printed side
- A3 black and white: 10p each printed side
- A3 colour: 50p each printed side
- A4 printed stills: 25p each
- Recorded to digital media: charge to be agreed in advance of the request. The minimum charge where an image needs to be obscured is £75.
- Postage: we use the current Royal Mail rates depending on the type of service you request.
- Courier: at the most cost-effective rate on the market dependent on the type of service you request.
