Care leaver's health and wellbeing support

Your health and wellbeing are important and we want to make sure that you have access to services to help you.

Once you turn 18 years old we'll provide you with a ‘health passport’ summarising your immunisations and current health needs.

Help to stay healthy for care leavers

Your health is really important to us. We'll make sure you have access to the services you need to keep you healthy.

We can:

  • Give you a 12-month gym pass to use at one of the leisure and sports centres in the borough of Rochdale. Find a leisure and sports centre
  • Help you to register with a doctor (GP). Find a GP
  • Help you to register with a dentist. Find a dentist
  • Explain your full health history using the health passport.
  • Get you access to specialist services if you need them, such as drug and alcohol services.

Mental health support for care leavers

Your mental health is just as important as your physical health. We want to make sure you know that there is always somebody here for you.

We can help you with:

  • Information about counselling services that are available locally
  • Issues that are important to your identity, such as religion, sexuality, gender or culture

Sexual health support for care leavers

We understand it can be a touchy subject, but sexual health is an important part of physical and mental health. We want you to know you can ask us for advice.

We can give you access to sexual health services like getting contraception or help with a sexually transmitted infection (STI).

Paying for prescriptions as a care leaver

As a care experienced young person, you're eligible for free prescriptions until you turn 25 years old.

This includes help with the costs of:

  • Medication
  • Eye tests
  • Glasses or contact lenses
