Need help with your claim?
Claiming benefit that you're entitled to is your
right. However, we're aware that some people feel
uncomfortable asking for assistance, or have difficulty filling in
forms.
The online form uses internet technology to make it quick and
easy for you to fill in and complete. It is interactive, and will
help and guide you to fill it in correctly. You will only be asked
questions that are relevant to your circumstances.

We may be able to visit you at home if you're elderly
or disabled, can't get out and need benefit advice. We realise some
customers have difficulties coming to our offices. To request a
home visit contact us.

Printing the declaration form
You'll need to read the declaration, print it, sign it
and return it to us. You can print this by going to the menu and
clicking on File then selecting 'Print'. If you don't have
access to a printer, you can
- Tell us when you fill in the online form and we will send you
any necessary pages.
- Print your declaration in your local library where we have
free internet access. Library locations and
opening times.
Evidences to support your claim
Once you've completed the online form bring all the necessary
information to any of our Customer Service Centres or post the
relevant information to the contact details on this page (please do
not send important document by post). If your form is complete
we'll fast-track your claim.
